New York State Certification of Birth - which number is the 'local registration number' and which birth certificate number

My husband is filling out a NYS Dept of Health Form DOH-4380, "Application for Irish Dual Citizenship". The form asks for both the Birth Certficate No. as well as the Local Registration No. He was born in 1944 in Albany. He has two documents for his birth:
1. Certificate of Birth Registration. (only 1 number typed on form)
The typed number is to the right of the title on top. Is this the local registration number?
2. Certification of Birth (2 numbers on this form)
On the top right, there is a field labelled 'No. H' followed by a blank line where a number is typed in. This number is different than the number on item 1 above, so we are assuming this is the birth certficate number. Below that number is a field labelled 'Registered No." The number typed in here is the same as the one on the Certificate of Birth Registration so we are assuming this is a local registration number and not the birth certificate number.

Can anyone verify which number is which on the 'Certification of Birth'
